Single Assessment Process - Using Assessment Tools in Practice

Targeted at Any member but of particular interest to multi disciplinary teams working with older people

The National Service Framework for Older People prioritises ‘person-centred’ assessment approaches, and much has been written about what actually constitutes person-centred care. The results from the literature to date seem to indicate that the Single Assessment Process (SAP) has yet to prove itself in achieving this aim.
